One district-wide file including all students in grade 12 who are currently active in the district, sorted by student. Note: This file will be used for pre-printing the Utah Senior Intentions Survey.

End of S.A.T. Testing

One district-wide file including all students who have been active in the district at any time during the current school year sorted by school-student name. This file must contain the verified disaggregation codes, and should be electronically sent to DCS immediately upon the completion of testing, to allow disaggregation code verification prior to scoring.

Clearinghouse
One district-wide file including all students active in the district as of October 1st and delineating Special Education Self-contained, Homebound, & Hospitalized students. Student enrollment by grade, gender, and race will be extracted as well as class size information for all CORE classes (K-6 by elementary classroom teacher and 7-12 by subject matter code).

Clearinghouse
One district-wide file including all Special Education students. Information on all students with IEPs will be extracted. Note: Extracted data will be forwarded to SARS electronically and hard copy verification reports of this data will no longer be available through DCS.

One district-wide file for each grade level (K thru 6), plus one district-wide file for each secondary Subject Area being Tested, sorted by school-class-student name. This file is due three weeks before the start of the test window.

Final Year End Enrollment / Membership Report
Clearinghouse
One district-wide file including all students active in the district at any time during the current school year. General student membership (S3), as well as specialized ATE class membership and statistical information will be extracted.
Finance and Statistics and Applied Technology
NOTE: Data that comes into the Clearinghouse is student specific data. In some cases, that individual student data is processed and uploaded to the appropriate department in a summarized format. It has never been the intent of the Clearinghouse project to print hard copy reports. Districts should verify the accuracy of their data before passing that information on to the Clearinghouse.
