This report is located in the Services and Programs module.

The Student Services application is used to track non-curricular services provided to students by the school staff, health professionals, or other providers. Examples of such services would be any type of therapy (physical, speech, behavioral), any type of counseling, tutoring service, or any meetings regarding the welfare of the student or dictating a plan of action for a student (such as an IEP). For each service administered, basic information is tracked about the type of service and the date and time it was provided. In addition, for each service rendered, an optional Recheck Date may be recorded. The Student Services Recheck Report will generate a list of Services with recheck dates. Administrators can use this report as a checklist for follow-up purposes.

In the default report, each Service with a recheck date is listed in chronological according to the recheck date for a defined date range. The names and ID numbers of students receiving each Service are listed as a sub-group under each Service entry. Various Service attributes of are listed, date of the original service, status of service, name of person providing the service, etc. Basic demographic data is included with each student name retrieved, including grade, gender, ethnicity, birth date, and age. The report is highly customizable through the selection of options, the application of select queries, or by rearranging the sorting order.

Quick, Step-by-Step Instructions

  1. Services and Programs | Reports | Student Services Recheck
  2. Click the Options tab and define options.
  3. Click the Select tab and define queries.
  4. Click the Sort tab and specify sort order.
  5. Click the Preview or the Print button.
  6. Click the Quit button to exit the report.
