SIS 2000+ Medical History
Quick Instructions

This report is located in the Health module and provides administrators with a list of health concerns (also called medical events or incidents) per student during a defined date range as recorded in the Medical History application in the Health program module. Each entry in the report displays the student's name and student ID number, concern name, gender, grade level, concern date, ethnicity, and alerts status. The report is highly customizable through the application of select queries and by manipulating the sorting order.

Quick, Step-by-Step Instructions

  1. Health | Reports | Medical History
  2. Click the Options tab and define options.
  3. Click the Select tab and define queries.
  4. Click the Sort tab and specify sort order.
  5. Click the Preview or the Print button.
  6. Click the Quit button to exit the report.
