11, Health Providers
The
Health Providers program is a database of the medical providers that are
used by the students of the school. Each student submits medical provider
information to the school during registration. The provider information
is entered into the Health Providers program and then assigned (linked)
to each student via the Health Information program (see
Sec. 2). The Health Providers program allows for multiple addresses
per provider as well as an memo field for notes. "Providers" will include
all types of health practitioners: doctors, dentists, clinics, HMOs, hospitals,
insurance plans, therapists, etc.
11.1 Open Health Providers
Click on the Providers button in the Health module's main menu
screen (or ALT+P on the keyboard). The initial screen (Fig. 41) will be
in the read-only mode (signified by the gray background color of the data
fields).

11.2 View existing providers
There are two sets of VCR buttons for scrolling forward or backward
through the records. Use the VCR buttons at the bottom of
the screen to scroll through the provider names. Use the VCR buttons
in the address area of the screen to view alternate addresses and
phone numbers for the selected provider name. These address VCR
buttons will be dimmed to gray if there are no alternate addresses to view.
11.3 Find an existing provider
Use the Find or List command buttons to initiate a search
for a provider (see Sec. 1 for more details on
the Find procedure).
11.4 Add a provider

Click on the Add button to open a blank Provider screen.

Fill in the data fields: Provider Name (last name, first name),
Type (from the drop-down list in the upper right), Address
Line1 and 2, City, State, Zip, Phone, Extension,
and Provider Notes.

Click on Save to confirm the provider information, or Undo
to abort

An alternate address may be added to the provider's record only after that
record has been saved with the primary address data. To add an alternate
address click the Add button in the address area of the screen.
This action will activate the address entry mode (Fig.42)

After entering alternate address data, click on the Save command
button to confirm the new data, or Undo to abort.

Notice that the VCR buttons in the provider's address area will
change from dimmed gray to black, signifying that there is more than one
address available for viewing.
11.5 Delete an existing provider or address

Select the provider name to delete using the VCR buttons or the
Find button at the bottom of the Providers screen.

Click the Edit button at the bottom of the screen. The edit mode
is on when the gray background color in the fields turns to white and a
blue line appears around the address area (Fig. 43).

Click on the Delete button at the bottom of the screen.

or , while still in the edit mode, select the provider address to
be deleted using the address area VCR buttons. Click on the Delete
button in the address area of the screen.

Click on the Save button to confirm or Undo to abort. If
no changes were made, the Save button will remain inactive (dimmed
gray)

11.6 Edit provider information
Find the provider to edit using the VCR command buttons or the
Find button (see Sec. 1 for more details
on the Find procedure).

Click on the Edit button at the bottom of the screen to edit the
name, type, address information, or notes for the provider.

Make modifications. Click on Save to confirm data, or Undo
to abort.
11.7 Exit the Providers program
Click the Quit button (or CTRL+Q on the keyboard) to exit the
program.
