• Birth Place -

The city of birth within the state, or the state of birth if out of state.
• Verification Basis -

Method of verifying identity and age of student - such as "birth certificate", "affidavit", "hospital certificate", "passport", etc.
• Birth Document # -

The serial number, if applicable, on the student's birth certificate or other verification document.
• Citizenship -

The type of citizenship of the student - such as "U.S. citizen", "resident alien", "illegal alien", etc.
• Country -

The student's country of origin.
• Seasonal/Temp Work -

Has the student changed school districts in the last 30 months due to seasonal or temporary work and therefore qualified for a Federally assigned number for migrant farm workers and their families? Answer "Yes" or "No".
• Marital Status -

"married", "unmarried", "registered domestic partner", etc.
• Membership -

Designate tribal membership, if applicable.
• User-Defined Information -

Click on this button to open a form containing additional student demographics data fields (Fig. 91). The fields in this form are defined, according to the needs of the district, in the udefstu table up by the System Administrator during the configuration of SIS 2000+.

• School Notes -

Additional general comments about the student.
• Change Password -

If the selected student is to have authorization to log on to the SIS 2000+ program, they must have a password. Define or edit the password as follows:
• Click on the field button next to "Change Password" (or ALT+P on the keyboard). This action will open the Change Password screen (Fig. 92).

• If there has been no password assigned to this student, skip the Current Password field and go to the New Password field, enter the password to be assigned, and enter it again in the Verify Password field.
• Click on Save to verify, or Cancel to abort.
• To change a password already assigned, enter the current password in the Current Password field.
• Enter the new password in the New Password field and enter it again in the Verify Password field.
• Click on Save to verify, or Cancel to abort.
• If the wrong Current Password is entered you will not be able to complete the Change Password procedure. Likewise, if your Verify Password does not match the New Password you will get an error message. If you have forgotten the Current Password you must refer to the System Administrator to recover it.
