Certificate of Compliance

The School Fee Certification of Compliance Forms are to be completed by all elementary school principles, secondary school principals, school district superintendents, and charter school directors/principals consistent with a 1994 Third District Court Order. By October 31 of each year, school district personnel and charter school personnel should compile the following information:

• Completed and signed School District / Charter School Certification of Compliance form.
• All completed and signed Elementary School Certification of Compliance form.
• All completed and signed Secondary School Certification of Compliance Form.
• Current School District/Charter School, School Fee Policy/School Fee Waiver Policy
• Current School District/Charter School, School Fee Schedule
