Emergency Preparedness Planning Guide for Utah Schools

Emergency Compliance and Assurance Reporting and Forms

Please Note: Although school emergency compliance and assurance requirements are still in effect, the detailed reporting required by USOE, of individual schools and school districts for FY13, will not be required to be submitted to USOE for FY14. Emergency related reporting for FY14 has been scaled back to resemble reporting prior to FY13, and is included in the annual "UCA Compliance and Assurance Form." This form covers information in addition to emergency topics, and is reported at the administrative level to USOE annually.  A copy of this form can be reviewed by selecting the link below. Because the detailed reporting is not required this year, the school and district level emergency compliance and assurance forms provided below can be used for location purposes to assist with compliance and may still be required at the LEA level, but, as noted above, are not required to be submitted to USOE.
