Visiting Team Reports
Every public secondary school and all Northwest accredited private and special purpose schools receive a site visit at least once every 6 years. Electronic versions of the visiting team report became available beginning with the 2002-2003 school year. Previous reports are available through the public library system.

Northwest Annual Report
This report is due October 15th of every school year.

  • Submitting the Annual Report

  1. Download the Annual Report that fits your school category.
  1. Complete report and mail to:
    Utah State Office of Education
    Attention: Georgia Lountensock
    250 E 500 S
    P.O.Box 144200
    Salt Lake City, UT 84114-144200

  2. Report must be received on or before October 15th. Send early.
