Visiting Team Reports
Every public secondary school and all Northwest accredited private and special purpose schools receive a site visit at least once every 6 years. Electronic versions of the visiting team report became available beginning with the 2002-2003 school year. Previous reports are available through the public library system.

Northwest Annual Report
This report is due October 15th of every school year.

1. Download the Annual Report that fits your school category.

• Utah Middle Schools
• Utah Residential Treatment
• Utah Special Purpose Schools
• Utah Supplemental Schools

2. Complete report and mail to:
   Utah State Office of Education
   Attention: Georgia Lountensock
   250 E 500 S
   P.O.Box 144200
   Salt Lake City, UT 84114-144200

3. Report must be received on or before October 15th. Send early.
