When is ... ?

[Following the inservice activity three things should be reported.

• A list of teachers who earn inservice credit should be sent to USOE, the list should include the name, SS # and District of the Teacher.
• If you are using the State Evaluation Forms it should be sent to USOE to be included in the statewide evaluation.
• Report of Inservice Activity should be sent to USOE if any funds to support the activity came from- USOE, Board of Regents Title II, District Title II, State Professional Development Funds.
