INTERAGENCY COMMON CLIENT REFERRAL FORM
The Interagency Common Client Referral Form, serves to refer customers to/from DWS, educational institutions, community agencies or advocacy groups for services.  Some examples include housing, food pantry, clothing, training and domestic violence advocacy. A release of information must accompany each referral that divulges personal information about the customer.  The original copy must be sent to the referred to agency either electronically, via mail or hand carried by the customer.  The copy will remain in the customer file as a record of referrals made.
