School Implementation Plan - Word

OverviewCTE Introduction Logo
An annual CTE Introduction School Implementation Plan is part of the CTE Introduction state funding requirements.  Each school receiving CTE Introduction funds must develop a plan and submit it to the district CTE director, so that all School Implementation Plans from one district/charter school can be returned to the Utah State Office of Education by October 15th.

Instructions
Read the form carefully and complete ALL sections fully, including teacher name, e-mail and qualifying endorsement.

  • Note 1: Indicate the schedule of rotations at your school.  If you present CTE Intro in any other format than trimester, you will need to officially request approval (form included).  Explain clearly how the CTE Intro program in your school is set up in order to receive approval from USOE each year to receive funding.  Please note that CTE Intro is designed to be a trimester course.
  • Note 2: The new form asks schools to indicate the grade level of students in the CTE Intro program. CTE Intro is a seventh grade course. Programs not in compliance may compromise funding.
  • Note 3: The CTE Intro Team will need to identify the trimester in which each standard and objective is taught with a check.  This is meant as a planning tool to help teams identify that all standards are being taught during the course.  This part of the form is in a simple checklist format.  Keep a copy of this Implementation Plan to use at the end of the year.
  • Note 4: The form has a second column which is grayed.  This is the End of Year Summary used as a verification of what was actually taught.    Keep a copy of the School Implementation Plan that you complete in the fall, and then complete the Year-End Summary section of the same School Implementation Plan in June.
  • Note 5:The Proposed Budget Summary Sheet is still a required component of the School Implementation Plan, and the Final Budget Sheet is submitted in the Year-End Summary.

Hard copies will need to be sent to Sherry Marchant at USOE with all original signatures and E-mail addresses.
