The CACFP forms come in two types, manual and automated. The manual forms are all in .pdf () format which require Adobe Reader to view. To obtain Adobe Reader click on the button below. Select manual forms are also in Excel format. () The automatic forms are in Excel format. A full version of Microsoft Excel is required to use the automated forms. The automated forms are not required by the USOE. You may continue to use the manual forms we provide or those that you have developed, if you choose, and they meet the same requirements.
