A candidate for the Driver Education Endorsement must fulfill the following requirements:

Complete the following as you apply for a Driver Education Endorsement:
- Print Driver Education Endorsement Checklist - PDF
- On the endorsement checklist, mark the boxes with an "X" to indicate that the minimum requirement in each area has been met.
- Attach a copy of your transcripts (original transcript for driver education courses) and highlight corresponding classes.
- Send the highlighted transcript, processing fee, and completed form to:
Endorsement submitted by individual— $35 processing Fee. Utah State Office of Education |
State Approved Endorsement Program (SAEP) (paid by district or charter school) $30 processing fee. Send to: Utah State Office of Education |
