Recommendations for a Smooth Application Process

Study all web site information carefully.

Register to take the appropriate Praxis II test(s) at the earliest possible date. Have scores reported to the Utah State Office of Education. Include test score sheet(s) or documentation of test registration in application package.

Assess your own qualifications by comparing your transcript(s) to the corresponding list of Utah qualifications.

Obtain any needed verifications of employment or institutional recommendations.

Complete and sign the application form.

Make check for $144.00 payable to the Utah State Office of Education.

Assemble and mail a complete application packet.

Respond quickly when background check materials arrive by mail.

Watch for preliminary approval and/or requests for additional information by email.

Follow your licensing process on CACTUS.

Update the Utah State Office of Education if your contact information changes during the process.

Mail a copy of test scores to USOE when they arrive to ensure prompt recording and approval.
