Reading Endorsement 2 may be attached to an Educator License with an Elementary, Secondary, or Special Education area of concentration.

Complete the following as you apply for an Advanced Reading Endorsement:
- Print Endorsement Form - PDF
- On the endorsement form, mark the boxes with an "X" to indicate that the minimum requirement in each area has been met.
- Attach your original transcript(s), and highlight corresponding classes.
- Send the highlighted transcript, $40 processing fee, and completed form to:
