Mathematics Endorsement Checklist - PDF

To teach mathematics in grades 7-12, a teacher must possess a current Utah Educator License with an elementary (up to the 8th grade) or secondary (6 -12) area of concentration and the correct mathematics endorsement. Acceptable lines of evidence for the endorsement include:

  • University or college work with grades of C or better
  • Approved professional development,
  • National Board Certification,
  • Clearly demonstrated competency

To apply for a Mathematics Endorsement, complete the following steps:

  • Print Mathematics Endorsement Checklist - PDF
  • Fill in the Name, Social Security Number or CACTUS ID Number, School, District, Home and Work Phones, Mailing and E-mail Addresses, Desired Endorsement, and SAEP Request blanks on the checklist.
  • Attach an original copy of your transcript/s with the corresponding classes highlighted and the course numbers entered into the checklist for the completed requirements.
  • Send the highlighted transcript, processing fee, and completed form to:

Endorsement submitted by individual— $35 processing Fee.
