Teachers who have a Utah arts teaching credential may apply for a Level II Specialist Endorsement in art, music, dance, or theatre at the elementary level. An elementary Level II Endorsement qualifies one to provide expert instruction for students in the school in the depth and breadth prescribed in the Fine Arts Core Curriculum, as well as resources for classroom teachers in Fine Arts strategies and materials for other curricular areas. It also qualifies one to receive training to become a mentor and inservice presenter for other teachers.

Complete the following as you apply for an Elementary Level II Endorsement in Dance, Drama, Music and Visual Arts:
- Print appropriate Endorsement Form (See above)
- On the endorsement form, mark the boxes with an "X" to indicate that the minimum requirement in each area has been met.
- Attach your original transcript(s), and highlight corresponding classes.
- Send the highlighted transcript, processing fee, and the completed endorsement portfolio to:
