The endoresment form incudes two levels of endorsement:
Level III - For licensed educators who desire to be hired as a specialist in Art, Music, Dance, or Theatre for a middle school. Applicant must have an elementary or a secondary license.
Level IV. For licensed secondary educators who desire to be hired as a specialist in Art, Music, Dance or Theatre for 6-12 students.

Complete the following as you apply for any Dance Endorsement:
- Print Endorsement Form - PDF
- On the endorsement form indicate that the minimum requirement in each area has been met.
- Attach your original transcript(s), and highlight corresponding classes.
- Send the highlighted transcript, processing fee, and the completed endorsement portfolio to:

State Approved Endorsement Program (SAEP) (paid by district or charter school) $30 processing fee. Send to: Utah State Office of Education |
