USOE - Licensing - Stage Craft Theatre

The endoresment form incudes two levels of endorsement:

Level III - For licensed educators who desire to be hired as a specialist in Art, Music, Dance, or Theatre for a middle school. Applicant must have an elementary or a secondary license.
Level IV. For licensed secondary educators who desire to be hired as a specialist in Art, Music, Dance or Theatre for 6-12 students.

Complete the following:

Secondary Stage Craft Theatre Endorsement Application.
On the endorsement form, indicate that the minimum requirement in each area has been met.
Attach your original transcript(s), and highlight corresponding classes.

Send the highlighted transcript, $45 processing fee, and completed application to:
