     1) Pharmacy Technician Certification Board (PTCB). The PTCB can be contacted at www.ptcb.org.
     2) Utah State Office of Education

Eligibility:     
-- Successful completion of a recognized Pharmacy Technician training program offered through a Utah    secondary school.
-- Obtain a Pharmacy Technician Certificate (CPhT) from PTCB.

Information:   
PTCB written assessment and certification fees are applicable.

Students must be a high school graduate and 18 years of age to be eligible to take the PTCB written assessment. The assessment is offered in March, July, and November. In addition, the externship must be completed under the direct supervision of a pharmacist.
